Program Form

School Information

We like to prepare for your experience at High Trails well in advance. Please take the time to fill this form out at least 30 days before your trip up to the mountains. Worried that your situation might change? If it does, just hop back online, adjust the form, and send us a new one.

Attending Adults

Please list below all of the adults that will be attending High Trails, as well as their position/relationship to your school. We require at least one certificated teacher from the attending grade level. We have lodging available based on a 30 student to 1 teacher ratio; please contact us if your needs exceed this ratio. Parents are not permitted as chaperones unless a special circumstance presents itself.
We do ask that you provide Email Addresses for these attending adults; we will send them out a Welcome Letter with High Trails basics for your trip, so that your entire team is on the same page and ready for a great stay up here. Thanks!

Medical Concerns

Do you have students with diabetes, life threatening allergies, or other similar medical situations? If so, we must know about all of them ahead of time. This allows us to prepare our staff and adjust our programs so that your students can succeed up here at High Trails. Please take the time to fully inform us of the situation and encourage parents to fill out our own Special Concerns Form if you think we need to talk with them individually.

Behavioral Concerns

Do you have students at school that have individualized behavior plans or programs designed to allow them to succeed in a learning environment? Do you have students that have aides or are in resource classes during parts of the school day? If so, we must know about all of them ahead of time. This allows us to prepare our staff and adjust our programs so that your students can succeed up here at High Trails. Please take the time to fully inform us of the situation and encourage parents to fill out our own Special Concerns Form if you think we need to talk with them individually.

Student Journals

We offer Student Journals for you to print and bring to High Trails if you choose. They are available to view here. Please tell us:

T-Shirts

We have one accessory for your students. T-Shirts are $10 each, and they are a great way to remember the experience for your current students and to showcase the event for next year's classes. Make certain to bring the completed T-Shirt Form with you when you head up to High Trails. If you would like T-Shirts, please fill out the following (you can change your order anytime by resubmitting this form):

Extra Food
Infirmary Supervision

We keep our costs down by asking for your assistance in supervising any students who have been checked into the infirmary during the day (8am-5pm). If students are checked into the infirmary during the evening (5pm-8am), we have staff on-call to provide supervision; this is included in the cost of tuition. If you prefer, though, we can schedule additional staff to be available to supervise your students in the infirmary during the day as well. We charge an additional $10 for each of your students attending High Trails, added on to your invoice, for this service.
